Chronic Disease Exercise Program
Beat disease with exercise!
Age, pain, disease and disability are all conditions that can be substantially improved with simple lifestyle changes. Only a very few health reasons should keep people from becoming more active.
Exercise improves:

- Balance
- Energy levels
- Pain control
- Quicker rebound from sickness or injury
- Performance of simple daily tasks
Patients must be referred by a doctor and qualify by meeting standardized criteria for each chronic illness.
- Patients enrolled in the exercise program must qualify by meeting the chronic disease criteria and be referred by a licensed healthcare provider.
- First Care's medical personnel will design an effective set of exercises individually designed for each physical condition.
- As the pateint progresses, the exercise program will be adjusted as needed.
- Progress will be documented through charted goals and monthly review of development/improvement.
- Updates on his/her patient progress will be available to the medical provider.

Two programs available:
- Monitored Exercise Program: A comprehensive program including education and an exercise therapy program. Patients are scheduled two to three times weekly, Monday through Friday and Saturday by appointment. They are continuously monitored by professional staff. All clients are required to begin their exercise reime in the Monitored Program. Progress is measured at 4 weeks and 8 weeks. If 8 week goals are achieved, staff, in consulation with the patient's provider, will consider moving patients into an independent exercise program.
- Intermittently Monitored Exercise Program is a comprehensive individualized education and exercise therapy program as well. These patients no longer require continuous monitoring, but they will still be monitored regularly. Progress will be reviewed, goals will be charted and documented and made available to the patient's provider. Patients enrolled in the Intermittently Monitored program will have access to the equipment whenever the Monitored Programs are not in session. Angela Dombkowski is our exercise trainer who works with the program and is avialble in the afternoons.
Insurance Coverage: First Care's Chronic Disease Exercise program is covered by some insurance plans.
